Community Access Facilitator

About the role:

The Community Access Facilitator plays a vital role in supporting individuals with disabilities to live active, independent, and fulfilling lives within their communities. The role is guided by a person-centred approach and the values of dignity, inclusion, and empowerment.

Qualifications:

Minimum Level 5 Certificate in General Health and Social Care, QQI (NFQ) or equivalent level in Health Care, Intellectual Disability or Special Needs Assisting qualification or relevant field.

At least one year of experience in an intellectual disability setting or similar health care setting working directly with health care clients.

Contract Type:

Full Time Post dependant on HSE funding. 

Closing Date: 5pm, Friday 15th May 2026
